General Case Management is a service which assists individuals who have a
mental health diagnosis with building safe bridges in the community.
Potomac Case Management Services utilizes a team-based model. In this
model the consumer is the integral member of the team. He/she will be encouraged,
supported, and expected to participate in and approve every decision of
their service plan. Each consumer is assigned a case manager, but he/she
will recognize a broader level of support with backup support from the
case management team as well.
What services are offered?
Each person’s situation is unique. The individual will assist in
completing their own initial assessment for which a service plan is joint
developed. That plan could include the following:
- Service planning
- Service linkage
- Monitoring of service delivery
- Consumer advocacy
- Crisis intervention
- Case coordination
- Family support
- Consumer outreach
- Re-assessment as progress is made

- Schools
- Mental health providers
- All service agencies
How can a referral be made?
If you wish to make a referral, please feel free to download our Case
Management Consumer Referral Form (PDF). We urge you to also download
the Referral
Guidelines for Adults (PDF). Please fill out all sections completely.
Once the referral is completed, you fax it to our office. However, the
original referral must be mailed to us as well. Please DO NOT e-mail referrals
due to confidentiality issues.
